Family history and cancer risk survey

Derbyshire Dales Primary Care Network is reaching out to patients aged 30 to 65 with a short questionnaire about family history and cancer risk.

The network is asking residents who receive one to take a few minutes to complete it.

Cancer that runs in families can sometimes be linked to inherited genes. By understanding your family history, your GP surgery can identify whether you might benefit from additional health checks or genetic testing.

The questionnaire takes just a few minutes to complete online using your mobile phone. Receiving it does not mean there is any concern about your health, it simply a routine proactive check being offered to patients across the Derbyshire Dales.
