Jeff King and Ryan Colclough have both been recognised for their excellent performances this season, helping the Spireites reach the play-off final.
The rest of the line-up is dominated, understandably, by Wrexham and Notts County players.
Phil Parkinson, who led Wrexham to the title, has been named Manager of the Season.
And Notts striker Macaulay Langstaff, who has scored 42 goals, a record for the division, all from open play, has been named the Player of the Season.

9. Macaulay Langstaff - forward
Just the 42 goals. All from open play. Not one penalty. History-maker. Photo: Cameron Smith

10. Paul Mullin - forward
A scorer of lots of goals, 38 of them, and some mightily important ones, too. Photo: Jan Kruger

11. Ryan Colclough - forward
The talented winger has got seven goals and five assists since signing from Altrincham, where he bagged seven in the first-half of the season, taking his tally to 14. His direct dribbling and ability to take on a man has made him a fan favourite with the Town fans. Photo: Tina Jenner
